NCQA confers PCMH status

After working diligently for over a year, we submitted our full application to the National Committee for Quality Assurance this fall and received certification as Patient Centered Medical Homes for both of our offices, Grace Family Medicine and Joy Family Medicine. This is not only an honor but an indication of our ongoing commitment to high quality patient care.

NCQA’s Patient-Centered Medical Home (PCMH) is an innovative program for improving primary care. In a set of standards that describe clear and specific criteria, the program gives practices information about organizing care around patients, working in teams and coordinating and tracking care over time. The NCQA Patient-Centered Medical Home standards strengthen and add to the issues addressed by NCQA’s original program.

The Patient Centered Medical Home is a health care setting that facilitates partnerships between individual patients, and their personal physicians, and when appropriate, the patient’s family. Care is facilitated by registries, information technology, health information exchange and other means to assure that patients get the indicated care when and where they need and want it in a culturally and linguistically appropriate manner.

2012 - A Busy Year

We’ve been very busy here at His Branches over the past year serving the families of Rochester in a variety of ways. From patient care to much needed activity for neighborhood youth and critical counseling in the area of life decisions, the staff and volunteers of HBI have been pouring their God-given passions into the work they do for their community. As our services to the community grow, we’re challenged with the reality of “doing more with less.” In order to maintain our level of quality patient care, we must be proactive in finding ways to increase our revenue in order to continue serving the families of our community.
